View preferences separated for index and messages

The cookies for the current view have been separated so that you can have a different type of view selected for the index than for viewing a message. For example, you can view the index in "classic" mode and select "flat" for viewing messages. Once you select these two views, they will remain the same (on that machine) until you change them. At the moment, there is still just one profile control for the view type.
